____ Visa - if not a U.S. citizen
____ College transcripts, proof of high school graduation, G. E. D., or equivalent experience
____ Copy of Certificate of Completion (if applying for 2nd year program from correspondence)
The following must be received by the school prior to your start date:
____ Two (2) recent, passport-size photographs
____ Completed Personal Recommendation Form
The following must be received six (6) months prior to your foreign missions trip:
____ Copy of signed passport (please see passport paragraph)
All paperwork in languages other than English must be translated and notarized before submission.
